the following persons submit a letter of reference prior to the deadline for
application directly to The Community Foundation of Frederick County, MD, Inc.:

Please submit an official college transcript with a minimum GPA of 2.5 on a 4.0
scale. The
information provided herein is accurate to the best of my knowledge. If I am
awarded the scholarship, I will abide by the terms of the scholarship and teach
in the State of Maryland.
